FAQs

  • No insurance is excepted. Current forms of payment is cash/credit card. FSA/HSA accepted as well.

  • You can email me at: me@bethanyvivo.com or call/ text 973-988-3755. Once we decide on a date & time. You will receive a link to book and fill out forms. This is vital to fill out forms before your appointment.

  • Preliminary paperwork is key. We go over and discuss your history, screen you from head to toe and in most cases begin treatment that same day.

  • Absolutely! I encourage any of your providers to contact me. Whoever the other provider is Trainers, PT’s, Chiropractors…etc. I love to collaborate so we are all on the same page as far as a treatment plan.

    We can also collaborate virtually, via email, to work together to bring you back to the things you love to do.

Policies

Arrival

Upon arrival you will be asked to wash your hands or sanitize.

Masks

Masks are optional and not required by law at this time.

Screening Checklist

Please reschedule if you are not in optimal health, experiencing any symptoms related to cold/flu, have tested positive for COVID.

Cleaning

We use a disinfectant to clean all common surfaces, handles, knobs, doors, treatment tables, faucets, etc.

Payment

Prepaid sessions give us less to physically handle in terms of cash and cards. A superbill maybe available if needed for reimbursement through insurance. All FSA/HSA payments are accepted.

CANCELLATION & LATENESS POLICIES

  • Current forms of payment is credit card. FSA/HSA accepted as well.

  • We have a 24 hours cancellation policy. Late cancellations or no shows are charged 50% of service fee.

  • Feel free to rebook, reschedule or cancel as needed 24 hrs before our appointment.

  • If you have a package/multiple prepaid sessions, any cancellation made less than 24 hours prior to appointment will be a forfeited appointment. No additional fee will be charged.
